Adult’s Social Worker
Team: Mental Health & Hospital Discharge
Location: Sheffield
Rate: £30
Based at Michael Carlisle Centre and across SHSC Trust work sites. The candidate will need to be an experienced social worker with a mental health background.
Core tasks and duties as follows:
Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
Some mental capacity assessments, BI decisions and safeguarding as required
Attending 117 meetings, ward rounds, professional meetings
Facilitating learning opportunities for ASYEs through shadowing
Participation in duty rota
